Damn, I hate choosing my children. I shall try, but I am sure there will be tears afterward. Please note, I base my choices on the memories that accompany the games, not just the games themselves. While the following are all from Nintendo, I am not a fanboy by any sense of the word, it's just the NES and N64 are the first two systems I have ever owned, thus happier memories.

5. Red Dead Revolver. I'm a sucker for westerns. Firefly is my all-time favorite show and with recent movies like 3:10 to Yuma, the genre has never been better. Unfortunately, there have been very, very few western games. Gun and Red Dead Revolver are the two that stand out for me, but Red Dead holds many memories for me. Probably not a common thing on a top-5 list, but I'll never forget it.
4. Final Fantasy VII Yeah, yeah, go ahead and call me cliche. Doesn't matter. FFVII changed what video games meant. (Along with games like Metal Gear Solid). The PS1 brought on a slew of games that were monstrous in every way, and this is the magnum opus of role-playing games in my opinion. They challenged the rules of games and how stories are presented. I hear all the time about people going against FFVII, and they can suck it. You don't have to like it, but you need to appreciate it.
3. Super Mario Bros. 3 My favorite type of games are platformers because of their pick-up and play abilities and accessibility. Everyone applauds the first Mario, but for me it was this third one that captured all the elements of a true to life perfect platformer.
2. The Legend of Zelda: Wind Waker The Zelda series has always been my favorite for the sheer quality of the games produced. Most hail OoT as the best Zelda, but for me, it was Wind Waker. I love all the Zelda games, and while Twilight Princess was an extremely amazing feat, the art style and different take on the story brought Wind Waker to the top of my list. Plus, its the ONE GAME that does water levels in video games right for the first time. I even wrote an article on that fact, but that's for another time...
1. Shadow of the Colossus Ah, Shadow of the Colossus. What is there to say? If you've never experienced this game, you need to. It's near and dear to my heart in every way possible and I can't really say into words what makes it so good. It takes games to a whole new level, much like FFVII did, IMO.

In no particular order, and probably leaving out some games I shouldn't, here are 5 games off the top of my head that made a huge impact on my life as a gamer:
1. Final Fantasy VII - I can't really say anything to adequately describe this game. I've played through it so many times, and it never loses its impact. Played it first in 1997 when it came out, and I'm playing it now in 2008 via my PSP.
2. Mega Man X - The perfect simple, straight-forward action game. Another game I continue to play over and over again, even now with my PSP's SNES emulator.
3. Resident Evil - I played this game far too young and it was the first game to ever scare me. It was also the first game I managed to completely break, being able to complete it in 40 minutes.
4. Half-Life - completely redefined what a shooter was supposed to be. Any decent shooter that has ever come after it pretty much owes its existence to Half-Life.
5. Super Mario Bros 3 - The game that sucked me into gaming for the first time when I was 4 years old. It was my first game for the NES I got for Christmas. It's still easily one of the best platformers of all time.

Looking back at my list, this really needed to be a Top 10, haha. I'd love to be able to add things like Diablo 2 and Command & Conquer.

5. God of war 2 It was an awsome sequal to the orginal which was great and this one pushed the story and added so many power ups
4. Metal Gear solid 2 I do't unterstand all the hate for raidin(sp) but maybe i am one of the only people that like him because metal gear solid 2 was my first metal gear game 3. Ar tonelico This is just a funny RPG it's filled with good musc a fun Dive system and it's filled with great story and lots of laugh's
2. Disgea This game was one a friend told me to check out and it has near and dear to me since i first started playing i fell in love with the crazyness of the charecters
1.Final fantasy 7 What can i say it was the first RPG i ever played and i fell in love with it

some honorale mentions that are definitely in my top 10 but didn't quite make my top 5. Legend of Dragoon was amazing so that definitely is up there and Shadow of the Colossus. (Ico may be on their too)

5. Call of Duty 4 4. Radiata Stories 3. The Legend of Dragoon 2. Chrono Cross (The Soundtrack alone makes it epic) 1. Final Fantasy VII (Yeah, yeah, it's what got me started though)

If Mom and Dad had gotten you a N 64 for Christmas that year instead of the Playstation, like you wanted, you never would have played Final Fantasy VII and might have never played Final Fantasy Tactics. Oh how different your life could have been.
